Fifth
Grade Discipline Plan
Rules:
Students breaking rules will be marked on the clipboard from each teacher.
1st and 2nd time- warnings written on the clipboard- (strikes)
3rd time- written mark on the clipboard (strike) and an action plan
Action plans are copied and sent home to be signed. Two action plans in a four-week period results in an office referral. If the student does not return the Action Plan the following day, the result will be a phone call to a parent.
Severe disruptions will be referred to the office immediately and teachers may choose to make phone calls even if an action plan is not earned.
Rewards:
Classes can earn Gold Rings for following rules. Classes will receive a reward when their goal is met.
Students may earn “Caught in the Act Slips.” Students receive slips throughout the week for good choices behaviorally and academically. At the end of the week, each teacher will draw five names for a reward.
Conduct Grade:
A student’s conduct grade is determined by the number of days a student receives a strike. For example, if a student earns a strike for 3 days during the nine-week period, then his/her conduct grade would be an S+. The formula is as follows:
S+ 0-4 days
S 5-10 days
NI 11-14 days
U 15+ days
*An action plan will automatically reduce a student’s conduct grade one letter grade.
*In school suspension will automatically reduce a student’s conduct grade one letter grade.
*A student earning a U in conduct for a reporting period (progress report or report card), will be required to have a parent chaperone on any field trips until the next reporting period.
*Students that continuously demonstrate unsatisfactory conduct will be required to have a parent chaperone on the end of the year field trip.
