Please have the following documents in hand in order to efficiently register your child at GMS: (*=Required)
1. * Proof of Residency – Utility bill, lease agreement, etc.
2. * Withdrawal Form - from previous school if enrolling during the school year
3. * End of Year Report Card – from the previous school year
4. * Copy of Birth Certificate
5. * Copy of Social Security Card
6. * Immunization / Ear, Eye, & Dental – Parents have 30 days to have Immunization records transferred to Georgia Form #3231 and 120 days to have Ear, Eye, & Dental transferred to Georgia Form #3300. This can be done at the Columbia County Health Department or with your personal physician.
7. Gifted or Special Education Information (If Applicable)
8. Standardized Test Data - Helpful in appropriate placement
9. Custody/Guardianship Papers (If Applicable)
