New Students must have the following documents:
Child’s Social Security Card
Child’s Birth Certificate (certified copy)
GA Immunization Form (#3231)
GA 3-point Screening form (#3300)
Parent’s proof of address (power bill/water bill/or gas bill)
A withdrawal slip and/or report card from previous school
Additionally, parents will be given a Brookwood registration packet to complete.
