  • Important News for Parents of 5th Graders!-
The State of Georgia has immunization requirements for students entering the 6th grade.

Your child may require additional immunizations for sixth grade to stay enrolled in middle school for school year 2010-2011.

If your child does not have the required vaccinations and a complete 3231 (State of GA immunizations form) on file at the school, your child will not be allowed to attend the first day of middle school for 2010-2011.

Most students entering sixth grade will need an additional Chicken Pox (Varicella) vaccination. Your health care provider or public health department can provide the needed vaccinations. You can obtain a copy of your child’s current immunization record (Form 3231) from the elementary school to take to your health care provider or public health department. Contact your school nurse with questions or concerns.

Please do not wait until the summer to get the immunizations.
