Proof Of Residency
(lease/purchase agreement; utility bill, etc)
Copy of birth certificate
Copy of social security card
Immunization records
Withdrawal form from previous school or report card
Copy of parent's ID
If parents are divorced, we need documentation on who has legal custody of the student as awarded by the court.
