| Camp Eagle's Nest Extended Care Program |

The Extended Care program operates daily before school from 6:30 AM to 8:00 AM. Students are released at 8:00 to go to their homerooms or to the breakfast program in the school cafeteria. After school care is provided from 3:30 PM to 6:00 PM. The program is offered on regularly scheduled school days. We are closed during the scheduled school holidays and on staff development days.

An annual registration fee of $25 per family is due upon enrollment. The Before School weekly fee per child is $18.00. The After School weekly fee per child is $40.00. These fees are for one week. If a child attends one day, the weekly rate will be charged to their account. We do not offer a drop-in rate per day. Children placed in After Care at 3:45 will be charged the weekly rate.
All fees are due by TUESDAY of the current week attending and payable by CHECK or MONEY ORDER - cash will NOT be accepted. Monthly payments must be made by the first Tuesday of the month. A $25 processing fee will be charged for returned checks. After two returned checks, only cash or money orders will be accepted. Written notice will be received for accounts past due one week. Service will be terminated for accounts two weeks past due and the $25 registration fee will be charged for readmittance to the program after the account is paid in full. |

If your child is injured seriously, first aid will be applied, 911 will be called and you will be contacted. Be sure to keep your phone numbers and emergency contacts current.

| Students should bring all books, paper, pencils and other necessary supplies to complete homework each day. Children are not permitted to leave the After School program and return to their classrooms at any time. |

| The drop-off and pick-up area for the Before and After Care programs is the cafeteria. Parents may enter through the covered walkway closest to the main lobby. For the safety of your child, parents MUST sign their child in and out daily. Your cooperation in this procedure is greatly appreciated. |
| Withdrawal from the Program |

| If you plan to withdraw your child from the program, please notify the coordinator in writing. |

| We encourage students in grades 1 - 5 to work on their homework assignments while they are attending the After School program. After eating a snack, students withdraw to a quiet area for homework time. The staff will be available for assistance. |

| Students will only be released to adults that are listed on the registration card. We will not allow your child to leave with anyone not listed. A signed note is required from the parent, if it is necessary for someone not on the card to pick up the child. The students must be picked up no later that 6:00 PM. There will be a $1.00 per minute fee, per child, for pick-up after 6:00. This should be paid to the supervising staff member at that time. After the third occurrence, your child will be withdrawn from the program. |

| The Martinez Tax ID number will be available in January to receive the Childcare Tax Credit. |

| The children enjoy the Camp Eagle's Nest Extended Care Program and normally behave very well. We treat the children with respect and encourage them to treat others with respect. We do not allow running indoors, destruction of equipment, foul language, etc. Disrespect, defiance and disruptive behavior will be dealt with immediately. A child may be reviewed for dismissal from the program for inappropriate behavior at any time at the discretion of the school principal and program director. |
