| The following information is required for registration: |

Georgia Certificate of Immunization (Ga. Form 3231)* |

Georgia Certificate of Eye, Ear and Dental Examination also known as 3-Point Testing (Ga. Form 3300)* |

Proof of residency (current gas, water or electric bill). |
|
•
|
Students social security card. |
*The required health forms may be obtained from private physicians, Fort Gordon medical personnel, or the Cola. County Health Department |
| Children, NEW to the Columbia County Public School System, are also requested to register for Grades One - Five from March 17 - March 26, 9:00 - 2:30. |
