No child shall be admitted to the public schools of Columbia County unless the child submits a valid Certificate of Immunizations. This certificate must reflect the official requirements at the time the child enters school, and the child will be subject to any updates of this certificate as specified by law. This certificate will be maintained throughout the child’s attendance in the school system. Students entering school without a Certificate of Immunization on file will be required to submit proof of immunization on Form 3231. Students currently enrolled with Form 3231 on file will be required to provide an updated 3231 certificate reflecting inoculation for two doses of varicella or history of disease and the second dose of measles, mumps, and rubella for entry into the kindergarten,sixth grade or any new student to Georgia regardless of grade level.
The only exceptions allowed are as follows:
1. Conflicts due to religious beliefs verified by parents notarized affidavit. (Download form at link above) 2. A justified waiver of 30 calendar days granted to residents of Georgia by the Superintendent or designee. 3. A waiver of 90 calendar days granted by the Superintendent or designee to students entering Georgia public schools from out of state, provided that a 3231 certificate is presented within 30 days with minimal intervals reflected in the expiration date to verify that vaccines have been started and can be completed in minimal time. 4. Medical exemptions are reflected on the 3231 and expire yearly.
Children whose parents fail to present or renew the Certificate within the time allotted shall not be permitted to continue in attendance.
