| | Click Here to Access Forms! | | Athletes must take the approved form to their doctor when they get a physical. Physicals are good for one year. In addition, athletes will need to provide a photocopy of their families health insurance card as well as a photocopy of their second semester report card. If the student attends summer school to earn credit, documentation of this must also be provided. |

This club is seeking members during the first year of operation and all proceeds will go towards projects that will have a lasting effect on the school. |
