Thank you for fully completing and submitting this form 24 hours prior to the start of the meeting at which you wish to speak. Doing so indicates that you will adhere to the following conditions as set forth in Policy BCBI, Public Participation at Board Meetings.

As outlined in Policy BCBI, all regular meetings of the Board shall be open to the public for observation and participation under the following circumstances.
