Health Care Plans

When a student is identified as having a medical condition requiring medical treatment and consideration an individual health care plan is developed with the school nurse, the student’s physician and the family.

The parent or guardian of a student with a chronic medical condition is required to provide the school with a Health Care Plan as soon as possible and have it updated yearly. Repeated failure of a parent or guardian to provide the school with a Health Care Plan and necessary medications may be considered neglectful and such neglect may be reported to the proper authorities.

Examples of medical conditions include but are not limited to:

Asthma   
Diabetes
Allergic reactions

As a safety precaution, students with chronic medical conditions that attend summer school should provide a copy of the Health Care Plan to the summer school principal.
